School Health Program

MEDICATION ADMINISTRATION

Medication should be given at home whenever possible. 
Please review the Medication Policy and Guidelines for Pulaski County Public Schools.  Any medication, including prescription and all over-the-counter require a physician order and a parent/guardian signature if they are to be given at school. Parents must provide the school nurse with a completed Mediation Authorization Form for any medication given at school. 

  • A new Medication Authorization From must be provided each school year and anytime there is a change in medication or dose time.
  • The first dose of any new medication must be given at home.
  • Your health care provider may fax medication orders directly to the school nurse.
  • Parent must bring all medication to school. 
  • For prescription medication to be given at school, please ask the pharmacist to divide the dose and provide a separate labeled container for dosage and prescription information. ** If mediation is sent to school in other types of containers (old bottles, baggies, paper bags) it will not be given.
  • Please note that cough drops cannot be given at school without a doctor’s order and parent signature.
  • Chap Stick and sunscreen may be sent to school for your child’s use only.  These cannot be shared with other students.

Self-Administration of Medication

Self –administration of any medication at school with the exception of asthma medication, insulin, or epi pens, is not allowed by any student.

    • Any self-administration of mediation for Asthma, Diabetes, and severe allergic reactions require a completed care plan signed by the health care provider and parent.
    • These care plans are good for only one school year.
    • You may talk with the school nurse about these plans.
